Symptoms

1. Create a domain template

2. Create a domain using the template with www prefix option checked.

If the template can be accepted the domain is created with all the required DNS records:

        domain.com.           A          10.55.53.18
        www.domain.com  CNAME   domain.com.

In case the template cannot be accepted the following warning message appears: 

Maximum number of additional Microsoft FrontPage accounts       Unlimited  0 
Maximum number of additional FTP accounts                                Unlimited  0

Ignore this warning and click OK.

Domain is created but without the record CNAME for www.domain.com:

   
        domain.com.      A       10.55.53.18
    
 

Resolution

This problem has been solved in Plesk version 8.1.1. Consider upgrading Plesk to the latest version.
